If you are using your motor vehicle to transport Service Users of Real Living Options Association Inc. you are required to keep your vehicle in a clean, tidy and roadworthy condition.
Failure to complete this document and return it by the due date may result in a cancellation of rostered work that requires you to use your own vehicle.
